A-to-E Assessments

The A‑to‑E assessment, also known as the primary survey, is a rapid, structured evaluation that clinicians use at the bedside to detect and treat immediate threats to a patient’s survival. It begins with Airway, ensuring the airway is clear and protected; then moves to Breathing, checking ventilation, oxygenation, and identifying any respiratory distress. Circulation follows, where pulse, blood pressure, and signs of hemorrhage are assessed, and life‑saving interventions such as fluid resuscitation or bleeding control are initiated if needed. Next, Disability evaluates neurological status through level of consciousness, pupil response, and basic motor function to spot acute brain injury or altered mental state. Finally, Exposure involves fully exposing the patient while maintaining warmth, allowing clinicians to look for hidden injuries, rashes, or other clues that could influence treatment. By progressing through these steps methodically, the team can stabilize the most critical conditions first, creating a solid foundation for any subsequent, more detailed secondary examination and ongoing care.
